Admission Documentation

Patients are assigned to a level of care upon admission. Each unit in Connect Care is configured to support specific levels of care. A higher level of care rate indicates that patients have spent more time in a unit with a matching level of care. Time spent in emergency departments as emergency inpatients is counted in the metric. Time spent in emergency departments prior to patients becoming inpatients is excluded.  

This Service can be entered in various locations including:

• Admit to Inpatient Order

• Various Activities access from the Unit Manager

  • Change of Care

  • Patient Update

• Event Management

Level of Care 

The metric calculates the total hours patients spent in department(s) with a matching level of care. It does this by calculating the sum of all ADT events where the following evaluates true:

• Patient Level of Care (I ADT 72) EQUALS ADT Allowed Level of CARE (I DEP 7065)

Level of Care

Description

• Proportion of admitted patients in units with appropriate level of care

Denominator

• For each patient WHERE Patient Class (I ADT 68) = Inpatient [101] OR Surgery Admit [130]: 

• SUM of all ADT event durations* WHERE Event Type (I ADT 30) INCLUDES any of the following: 

  • • Admission [1] 

    • Discharge [2] 

    • Transfer In [3] 

    • Transfer Out [4] 

    • Patient Update [5] 

• Exclude events prior to patients becoming an Inpatient Patient Class 

• Exclude Multifunction Units (MFU’s) 

*Event duration = Effective Instant (I ADT 59) MINUS effective instant of preceding event 

Numerator

• Total hours patients spent in department(s) with matching level of care

• SUM of all ADT event durations WHERE Patient Level of Care (I ADT 72) EQUALS ADT Allowed Level of CARE (I DEP 7065)

Calculation

• Numerator DIVIDED INTO Denominator expressed as PERCENTAGE rounded up to nearest integer percentage point

• Hospital Specialty Service: ADT 70 - CATEGORY - (e.g., General Internal Medicine [106])

• Hospital Unit: EPT 18880 - CATEGORY - inpatient unit/ward (e.g., EDM UAH WMC 5D4 GIM [101094229])

• Hospital Facility: EPT 18883 - CATEGORY - (e.g., EDM WMC University of Alberta Hospital [101094])

Validity

Accurate data relies on manual documentation when a patient no longer needs acute care services by the most responsible healthcare practitioner.  Department build configuration for level of care in Connect Care may not be fully complete or accurate.
