Admission Documentation

Patients are assigned to a hospital service upon admission. Each unit in Connect Care is configured to support specific hospital services. Higher localization rate indicates that patients have spent more time in a unit with matching hospital service. Time spent in emergency department as emergency inpatient is counted in the metric. Time spent in emergency departments prior to patients becoming inpatients is excluded.  Departments without Services are excluded, as well as Multi-Function Units (MFUs).  

This Service can be entered in various locations including:

Hospital Service Localization

The metric calculates the total hours patients spent in department(s) with a matching service. It does this by calculating the sum of all ADT events where the following evaluates true:

• Hospital Service (I ADT 70) EQUALS ADT Services (I DEP 7020)

Patient Localization - Hospital Service

Description

• Proportion of admitted patients in units with appropriate hospital service 

Denominator

• For each patient WHERE Patient Class (I ADT 68) = Inpatient [101] OR Surgery Admit [130]: 

• SUM of all ADT event durations* WHERE Event Type (I ADT 30) INCLUDES any of the following: 

  • • Admission [1] 

    • Discharge [2] 

    • Transfer In [3] 

    • Transfer Out [4] 

    • Patient Update [5] 

• Exclude events prior to patients becoming an Inpatient Patient Class 

• Exclude departments with blank ADT Services (I DEP 7020) such as procedural areas (Operating Rooms, Cath labs, etc. 

• Exclude Multifunction Units (MFU’s) 

*Event duration = Effective Instant (I ADT 59) MINUS effective instant of preceding event 

Numerator

• Total hours patients spent in department(s) with matching service 

• SUM of all ADT event durations WHERE Hospital Service (I ADT 70) EQUALS ADT Services (I DEP 7020) 

Calculation

• Numerator DIVIDED INTO Denominator expressed as PERCENTAGE rounded up to nearest integer percentage point

• Hospital Specialty Service: ADT 70 - CATEGORY - (e.g., General Internal Medicine [106])

• Hospital Unit: EPT 18880 - CATEGORY - inpatient unit/ward (e.g., EDM UAH WMC 5D4 GIM [101094229])

• Hospital Facility: EPT 18883 - CATEGORY - (e.g., EDM WMC University of Alberta Hospital [101094])
